Age Requirement

Kindergarten students must be 5 years old on or before September 1. Transitional Kindergarten student's birthday must be between September 2, 2018 and September 1, 2019. 

Required Information

Verification of the following required information must be presented to the office staff at time of registration for your child/children:

1. Parent/Guardian picture identification

2. Documentation of your child’s age

• Birth certificate, passport, or baptismal certificate
• Current proof of residency within Park Western Place Attendance Area using one of the following:
  • Southern California Gas Company bill
  • Department of Water and Power bill
  • Bank checking statement
  • Rental or lease agreement
  • Child support or social services check stub
  • Tax documents
  • For military families: Housing Verification document

3. Current immunization records for the following. Immunizations must be current and listed on record presented. Child cannot start class without updated immunizations. • Polio: 4 doses at any age or 3 doses with last dose on or after age four.
• DTP (Diptheria, Tetanus, Pertussis – Whooping Cough): 5 doses at any age or 4 doses with last after age four.
• MMR (Measles, Mumps, Rubella): 2 doses, first after 12 months of age and second dose 1–3 months later.
• Hepatitis B: 3 doses, second dose is given 1–2 months after first dose, third dose is 2–6 months after second dose.
• Varicella – Chicken Pox Vaccination: 1 dose after 12 months of age, 2^nd dose at any age after 1^st dose. 

4. Dental health checkup. Required for Kindergarten and 1^st Grade Only

Note: A physical examination is required by the time your child enters the 1^st grade. This is a California State requirement.
